NOTES:
1. A member of the Church may only vote on items 3, 4, 5 and 8 above by appointing the Chairman of the ACM as the member’s proxy to vote at the meeting. This is done by depositing with the Church an instrument of appointment, also known as the Proxy Voting Form, which has been embedded into the registration link below and enabled for only communicant members to vote (non-members can only register) by 30 December 2024. Upon submission, the combined Registration and Proxy Voting Form will be automatically sent to [email protected]

2. Attendance at the ACM (including Q&A) will be in person only. All communicant members are strongly encouraged to be present at the ACM.
3. Members who do not have email access will receive the Registration/ Proxy Voting Form by post, and will have to mail the completed form back to: ACM Secretariat, Mount Carmel BP Church, Clementi Bible Centre, 152 West Coast Road, Singapore 127370. It must reach the church office not later than 30 December 2024, 2359 hrs.

5. Any other matter to be raised for discussion at the ACM should be submitted in writing to the church office by Saturday 30 December 2024. All substantial questions will be addressed by the Church leadership in-person at the ACM.
